  2. In the “Message” area, simple write, “Smiles For Life”, then type any details that you like relative to the best times for your whitening appointment. For example, you might write: “Smiles For Life. If possible, I prefer a whitening appointment in the afternoon, on a Wednesday, Thursday, or Friday.”
  3. After you submit the form, our Smiles For Life office coordinator, Kelly Larson, will contact you directly to make your appointment!
